I've hunted for this a few times now, and can't seem to find Unity Remote 4 on the App Store.
Clicking on the link in the docs takes me to the app store - and then the App Store says the application doesn't exist (deprecated).
I'm a bit confused on this one. Also - I'm on the Australian App Store - not sure if this is US only...
Any clues would be really appreciated! :)
